To complete your Gift of Securities, you will want to provide the following information to your financial institution. The purpose of the fillable section of this form is to notify DePauw of your anticipated Gift of Securities and to indicate the designation of your gift. Privacy statutes may prevent your financial institution from providing us with any personal information.

Alternatively, you may provide this form (online or PDF ) to your financial institution and ask them to complete.
